Abstract

Prenatal ultrasound is a radiological examination that allows optimal follow-up of pregnancies. However, its implementation remains limited in poor countries due to a lack of equipment and trained health workers, such as in Burkina Faso. The aim of this work is to set up an ultrasound tele-expertise system. To achieve this objective, we mobilized human, material and IT resources. The design of the tele-expertise application was based on a generic open source software called “MedShakeEHR” that we have adapted to our context. The application runs in a network on a Linux system. It enables ultrasound data exchange and sharing with a remote expert for interpretation using the DICOM protocol. This device thus offers the possibility to pregnant women to carry out their prenatal ultrasound locally. It also allows the constitution of prenatal ultrasound database according security and confidentiality standards.

Il a été possible de mettre en place une solution simple et efficace (et apparemment ne nécessitant pas de moyens couteux et complexe) pour répondre à une problématique de santé publique bien concrète sur toute une population car, MedShake EHR/EDC et Orthanc sont des solutions libres. De ce fait, elles ont pues être récupérées, comprissent clairement et adaptées au contexte sans imposer limites, contrites et restriction technique aux acteurs du projet.

Maintenant, on peut se poser les questions suivantes :

  • À quoi aurai ressemblé la réalisation d'un tel projet, si seul des solutions propriétaires avait été mise à disposition et à quel coût ? Est-ce que seulement, il aurait été possible de le réaliser ?

  • Aurait-il eu la même capacité remplir sa mission après des populations les plus enclavées et pauvre comme présenté dans l'article ?

  • A combien valorise t'on ces solutions libres dont fait partit MedShake EHR/EDC et leur potentiel à rendre de tels services ? Qui pour investir sur MedShake EHR/EDC et faire en sorte que ce genre de projet reste possible ?

Dans l'article, il est fait mentions de plusieurs modifications sur les sources de MedShake EHR/EDC (sur la base et le module modGynObs). Des choses qui semble bien utile comme l'algorithme d'identification unique du patient et l'intégration de la base de donné IDC-10. Ok c'est très cool ! ... Elles sont ou les sources ? Peut ont les intégrer dans la branche principale de MedShake EHR/EDC ? Pas de trace dans le différent fork github ni de mention dans l'article. Dommage...
